Here you will find a break down for a typical initial Nutritional Therapy Assessment along with the necessary first steps to begin the process. Sessions vary based on the individual client’s needs and goals but one principal remains consistent. The key to a happy and healthy life is living authentically as your true self, the foundation of which is good nutrition.
Nutritional Assessment Steps
- Complete the New Client Form
This form is a standard new client questionnaire providing us with the information necessary to begin preparing for your nutritional therapy session. This is a secure form only viewable by Authentic Self Nutrition™ staff. We do not share or sell your personal information. (view our Privacy Policy here) If you feel uncomfortable providing this information over the internet, you may call our office at (503) 819-0771 or toll free (888) 505-1264. - Complete a Nutritional Assessment Questionnaire (NAQ)
Once we have received your New Client Form, you will receive an e-mail invitation to complete an online Nutritional Assessment Questionnaire. The purpose of this questionnaire is to create a benchmark for your “current” physical and nutritional health and takes about 15-25 minutes to complete. - Schedule Your Session
Once you have completed the New Client Form, Nutritional Assessment Questionnaire and Food Journal, we will contact you directly with any questions and schedule your personal session with Margaux J. Rathbun, B.S., N.T.P. - Complete a 3 Day Food Journal (Optional)
This is not necessary for an appointment but the more information we have the better we can investigate symptoms.
Using the provided forms or one of your own, tell us what you have eaten over a three day period and tell us how this impacted your mood or any other notable reactions.
What to expect from your nutritional therapy session.
- Comprehensive Nutritional Assessment Questionnaire (NAQ)
The nutritional assessment evaluates the foundations of nutrition: Immune System, Kidney & Bladder, Cardiovascular, Women Only Issues, Men Only Issues, Thyroid, Pituitary, Adrenal, Vitamin Needs, Sugar Handling, Essential Fatty Acids, Mineral Needs, Large Intestine, Small Intestine, Liver & Gallbladder, Upper Gastrointestinal.

The results of your nutritional assessment may expose various deficiencies in your diet. Authentic Self Nutrition™ takes a “Food First” approach to bringing your health back to your ideal state of balance. With a comprehensive food list detailing beneficial foods and various food sensitivities, we are able to answer the question, “What should I eat?”

The first session begins at $135 which includes all initial paperwork, review and initial written follow-up. Follow-up sessions are then $85 each with various packages and programs available depending on personal needs.
